I would say, that as the combat table is a pretty abstract representation of the engagement, it isn't a case of one round is one attack as such, but a series of maneuvers.
So, yes, a K result is the same as if the hero was fighting a single foe.

No. As long as we can agree, can be any class. Obviously the more information available on a particular area, the more we can 'bring to life' the world around the character.
